SmartSmile by Dental Health Services offers the lowest cost, and has no annual benefit max, no waiting period, but has a limited network of dentists. Click here to see the directory.
LifeMap Dental allows you to go to any dentist, but has a 12 month waiting period on most benefits. It has a $750 annual benefit max that increases $250 after every year until reaching $1500 in year 4. The coverage LifeMap Dental pays towards treatment increases each year as well (80% for preventative in year 1, 90% in year 2, 100% in year 3). Click here to see the benefit summary.
Denali Dental allows you to go to any dentist (choose the indemnity option - not PPO) and offers a $1500 Annual Max for their Basic Plan, but has a 12 month waiting period for Major care, and a 3 month waiting period for Basic care. This plan also has a $100 lifetime deductible. It pays 100%/80%/50% based on Usual and Customary charges.
United Concordia offers several dental plan choices ranging from low cost basic preventative plans to very comprehensive coverage with up to $2000 annual maximums. The most popular plan is their 501 plan that has a 6 month waiting period and pays 60% up to $1500 for basic and major procedures. Click on the link above to view all the plan options in detail.
